The Post
School Services – Adult Lifestyle Support Program commenced in
April 1997. The program was developed to respond to the
transitional support needs of young people with severe disabilities
who are exiting the school system as they begin to make the change
from being a student to becoming a young adult.
Post School Services has a
dual focus as it provides support not only to the young person but
also to their families / carers.
The Program aims to enable the young person to experience a range
of options and opportunities as they work toward the transition of
adulthood.
To be eligible for the
program, a young person must:
- Be 18
years old by 31 January in the year they enter the PSS program;
and
- Be exiting
or have exited special school or special education programs and/or
services; and
- Have a
level of disability which results in high and complex support
needs; and
- Be
eligible for assistance under the Queensland Disability Services
Act, 1992; and
- Not have
access to tertiary education, vocational training or employment
options.
 Exceptional circumstances: People may be considered
eligible for assistance  through
the Program under exceptional circumstances where they meet
the above criteria with the exception of (1) being 18 years
old when entering the Program, and/or (2) exiting special education
programs and/or services.
As a provider of services
under the Disability Services Queensland funded Post School
Services—Adult Lifestyle Support Program,
IMPACT Make Your Mark
will:
- Work
within the framework of the Queensland Disability Service
Standards
- Negotiate
with the young person, their family and the Supports Facilitators
an appropriate support package for the young person
- Provide to
the young people a service in accordance with program and funded
guidelines, which supports the achievement of the identified goals
of the young person
- Provide
ongoing monitoring of support packages provided to all Post School
Services participants
- Provide
information to the Support Facilitators during reviews of the
individual programs
