About the role: 

Transition to Work is a federally funded employment services contract for young people on their journey to employment and/or education. You will provide intensive case management by:

How will you do this?

  • Ensure that the requirements of the program and expectation to be actively engaged in seeking work or actively preparing for seeking work is understood by the participant
  • Identify and coordinate delivery of assistance for participants that will improve their employment and education prospects – through building self-esteem, facilitating access to non-vocational interventions and access to activities at the site and job matching
  • Positively impact on participants by assisting them into sustainable employment. Provide Post Placement Support to ensure employment and education sustainability
  • Develop and maintain relationships with community and local support organisations to ensure a holistic case management approach
  • Review Plans on a regular basis to ensure all activities and job search requirements are current and compliant
  • Document all mutual obligations in the Government IT system, and case notes in 3rd party software
  • Ensure compliance with the TtW 2015-2022 Deed, all supporting Guidelines, the Service Guarantee and the Service Delivery Plan
  • Ensure that all KPIs are met

What you need to be successful in this role:

Essential -

  • Understanding of working with people with complex social and employment barriers
  • Demonstrated ability to effectively communicate and engage with jobseekers from diverse cultural backgrounds
  • Demonstrated ability to effectively facilitate small groups
  • A proven track record of achieving performance targets
  • Experience in working in a contractually compliant (or similar) environment
  • Proficient IT and administration skills
  • Highly developed computer skills with proficiency in Microsoft Office applications, particularly, Word and Excel
  • Ability to work independently and in a team environment
  • Demonstrated ability to deliver excellent customer service
  • Queensland Driver’s License / C Class 
  • Possession of, or willingness to obtain, NDIS Worker Screening Card, Working with Children (Blue Card), National Police Check (AFP)

Desirable –

  • Experience in case managing young people aged 15-24
  • Certificate IV in Employment Services
